Professional Registration

Massachusetts General Laws require all physicians conducting the practice of medicine, podiatry, optometry, dentistry and/or electrolysis within a community to register with the Town Clerk.  This is a one-time registration.  If you practice in multiple communities, you must register with each city or town clerk.  A fee of $50.00 and a copy of the physicians certificate or license must accompany the applicable form.
